Per-graft pricing usually costs less, because you pay only for the grafts you receive. A package saves money only if your real graft count is above its break-even point — package price ÷ per-graft rate. Below that, you are paying for grafts you will never get. Under either model, get the graft count in writing.

Indian hair transplant clinics use four pricing models. Most quotes you receive will be one of these:

Per-hair pricing is a presentation trick rather than a genuine fourth model. A graft carries about 2 to 2.3 hairs on Indian scalp hair, so "5,000 hairs" is only around 2,200 to 2,500 grafts. Convert every per-hair quote to grafts before comparing it.
Because each model gives the clinic a different financial reason to bend your plan. Neither is dishonest by default, but you should know which way the pressure runs.

The over-harvesting risk is the one patients underestimate. Your donor area holds roughly 4,000 to 7,000 grafts for life, and extracted follicles never grow back. Five hundred unnecessary grafts today are five hundred you cannot use if your hair loss progresses at 45.
Only above the break-even graft count. Work it out in one line:
Break-even grafts = package price ÷ per-graft rate you could get elsewhere
Example: a ₹1,80,000 package "up to 3,000 grafts" against a clinic charging ₹70 per graft. ₹1,80,000 ÷ ₹70 = about 2,570 grafts. If you need fewer than that, per-graft is cheaper. If you need more — and the package genuinely delivers them — the package wins.

All figures are before GST, which applies at the same rate to both models. Two things follow from the table:
• Packages are priced around the middle of the market. Most men considering surgery are Norwood 2 to 3 and need under 2,500 grafts — exactly the range where packages cost more per graft.
• The package only wins if you actually receive the full count. A package that places 2,200 grafts while advertising "up to 3,000" has cost you ₹82 per graft, not ₹60.

No — because there is no such thing as unlimited grafts. Two limits apply to every patient, whatever the package says:
• The daily limit. Most clinics safely place 2,500 to 3,500 grafts in one day, keeping the time grafts spend outside the body short.
• The lifetime limit. An average donor area can safely give 4,000 to 7,000 grafts in total. Take more and the back of the head looks visibly thin.
"Unlimited" really means "as many as we decide to place on the day, at one fixed price". Since the clinic keeps more money the fewer it places, the word works against you. If a clinic offers an unlimited package, ask for the minimum number of grafts it will guarantee, in writing. If it will not give one, treat the offer as a marketing headline rather than a price.
Ask this before you sign. It is the question most patients never think of.
During extraction, a small share of grafts is damaged (transected) and discarded. After extraction, grafts are sorted and counted under magnification before implantation. A patient quoted 2,000 grafts might have 2,080 extracted and 2,000 implanted — or 2,000 extracted and 1,920 implanted.
The fair standard is billing on grafts implanted, because that is what grows. Per-graft quotes should say which count they use. If the answer is "extracted", you are paying for any grafts the clinic damaged.
You cannot count grafts in your own scalp, so the evidence has to come from the operating theatre. Ask for all of these:
1. A graft count sheet on the day, showing grafts by type — single-hair, two-hair and three-hair — and the total implanted, signed by the operating doctor.
2. Photographs of the sorted graft trays before implantation. Many clinics take these routinely; ask in advance.
3. The final count on your invoice, matching the count sheet.
4. The name of the doctor who performed extraction and implantation, on the discharge summary.
A clinic confident in its count will give you these without hesitation. When you look at any clinic's before-and-after photos — including our results gallery — check that each result states the graft count too.
Sometimes this is legitimate. Donor density, hair calibre or the true extent of thinning can look different once the scalp is shaved and examined closely. But it is also the most common way a per-graft quote grows after you have taken leave, travelled and sat in the chair.
Example: a quote of 2,000 grafts at ₹70 is ₹1,40,000. If the plan becomes 2,600 grafts on the day, the bill rises to ₹1,82,000 — ₹42,000 more, before GST.
Protect yourself before surgery day:
• Agree a written cap, for example "no more than 2,200 grafts without my written consent".
• Agree the rate for extra grafts in advance, so there is no negotiation in the operating theatre.
• Know that fees should be clear before treatment starts. Indian medical ethics regulations require doctors to announce their fees before rendering a service, not once an operation is under way.
Often, yes. Tiered pricing charges one fixed price per graft band — for example 1,501 to 2,000 grafts. It removes most of the incentive to add a few hundred grafts, because small changes do not move you into a new band, and it still ties price to the work done. The weak point is the band edge: a plan of 2,050 grafts can cost the same as 2,500. Ask what you get for the top of your band, and whether the plan is being nudged across a boundary.
Packages frequently bundle extras — PRP sessions, medicines, shampoo kits, hotel stays, airport pickups. These are worth something only if you would pay for them anyway. To compare fairly, strip every quote down to the same list: surgical fee, grafts, GST, blood tests, medicines, first wash, follow-ups. Put the extras to one side and compare what remains.
A package worth ₹1,80,000 that includes ₹25,000 of PRP you did not plan to have is really a ₹1,55,000 surgery plus an upsell.
Three things decide whether you get value for money, and none of them is the pricing model:
1. Who holds the instruments. Low-cost clinics often have technicians perform extraction and implantation while the doctor only marks the hairline. That affects graft survival and naturalness more than any price difference.
2. Whether the graft count is right. Too few leaves a thin result; too many wastes donor supply you cannot replace.
3. Whether every number is in writing before you pay a deposit — graft count, rate or package minimum, GST, inclusions and the cap.
